Minnesota Vision Therapy Center is a national leader in the diagnosis and treatment of functional vision disorders and the treatment of academic problems caused by these disorders.   

INITIAL EXAMS 

The first step in setting up an exam is to take the SYMPTOMS CHECKLIST and record the score which is necessary when setting up the appointment. If you are referred by your Optometrist please have the records faxed from your most recent eye exam to our office prior to your exam. 952-844-0810 (FAX) 

All initial exams are done in the Bloomington office.  The exam process is set up with 2 separate appointments. If the individual is under the age of 18 we recommend both parents attend both appointments in order to fully understand all information discussed. 

EXAM APPOINTMENT

• Arrive at the Bloomington office 15 minutes prior to your scheduled appointment.
• We require that all paperwork is completed prior to your arrival. Please click on the links below and print out the required forms.

• We do require that you bring your child to the exam without other siblings.  If siblings are brought to the exam you will need to reschedule your appointment as this distraction affects the results of the exam.
• If you arrive late for the exam you will need to reschedule your appointment for a later date.
• On the exam day we will complete all tests and you will meet with the doctor.  You will be given preliminary information about Vision Therapy.

 CONSULT APPOINTMENT

• The consult is for the parents only. 
• We will review the exam results and discuss program guidelines.
